Internal audits - Memory is a strange thing, especially when there are significant costs involved. Although everyone strives for a well oiled machine, it does not always start of that way, it can take many iterations and modifications to achieve that special product. Nobody likes to have what they have done checked as we are all perfect, however with age, lunch time drinks and a host of other distractions it can happen. This is why Internal audits are required, if things do go wrong from time to time, find out the cause and work out how to address the problem, "anyone can make a mistake only a fool makes the same mistake twice". . The question is who is the fool, the person who makes the mistake of those that manage the operation. As Henry Ford famously stated "the only real mistake is the one from which we learn nothing".
